NFS consistency problem?

No, you aren't. I did report that behaviour some time before (have to
check the eMails for finding it).
The clients ran 2.6.16.28 and we are seeing it once a week on one of 40
machines (mounting with noac option). Touching the file helps sometimes.
The server file system IS XFS. In the case of that problem I recommended
to access the "wrong" file via NFSv3.

Actually we are in the transition towards 2.6.21 on all machines
(servers and clients) and I don't have reports about the file update
errors, but it's too early to say "it disappeared".
