[pnfs] Added changes to 4.1-sessions branch, and merged with master branch

during the merge of the new sessions code with the master pnfs code, i
encountered changes to fs/nfsd/nfs4xdr.c:decode_exchange_id() that decode
the exchange flag and the implementation id which belong in the sessions
branch. so i reverted the master branch to match the 4.1-sessions branch,
and will check in the server decode_exchange_id() changes to the sessions
branch and do another merge with the master tomorrow morning.
