[pnfs] 4.1-sessions and master merge complete

the following patches were applied to the 4.1-sessions branch - the fixed
two bugs in the server op_sequence processing, and updated the server
exchange_id xdr.

then the branch was merged with the master. i have not tested the master - i
will do so today.
