[pnfs] Hang noticed during large file commit

Hi,
We were noticing a hang for files larger than 200MB. The cause turned
out to be a case of allocating a slot and not releasing it. These
patches fix that. It works for our prototype. 
Marc, could you please try these patches? It affects the commit through
MDS path too.
