[pnfs] Callback patches - first set

Hi Guys,
Here's the first set of patches for the callback channel. I'd like to
stage them into 3 sets so that they're easy to review:
1. General patches: These are largely independent of each other and
apply before the other changes come in.
2. Client side patches
3. Server side patches.
 
The current set is a set of general patches. They're small, fairly
obvious and should have no objections IMHO.
