[pnfs] [PATCH] 0/3 Update v4.0 stateid, sequence id and clientid for the draft 13 4.1 server plus more

I applied these three patches to the 2.6.18.3 pnfs git tree. 2 are from Mike
Sager, one from Marc Eshel

Ricardo - the first two patches also apply cleanly to the
linux-pnfs-2.6-latest tree.
