[pnfs] pnfs-gfs2 patches, take 3

	hello, benny,

	since a bug or two got fixed, i decided it was probably better to resend
these rather than use a squashme patch.

	changes in this take into account dean's and your comments:

	- strip code for handling >1 fh in a LAYOUTGET response
	- fix a bug in DS list handling 
	- fix error return values
	- tidy the ".8.1 = port 2049" comment
	- strip unnec. gpfs mention

	tested with 2 DSes, OK with small and medium I/O.
